I give permission for my son/daughter, __________________________________________, to

participate in the Corn Maze field trip on October 4, 2008 which I understand may require them to ride in a privately owned vehicle.

I also give permission for any adult leader of Elvaton Church or their representative to seek medical attention for the above named student in case of an emergency.

I further agree to release and hold harmless Elvaton Baptist Church, its officers, and/or any EBC adult responsible for child care, paid or volunteer, from all liability for negligence while acting in good faith.
